Online referral form

Please be aware that unfortunately due to funding constraints, not all mental health services are available in all areas of Lancashire. (For example we are currently unable to offer mental health support in Blackburn). You can check eligibility by visiting this page. If we are unable to help we will always try to signpost you to another service. If you think there is a service you can access in your area, please enter your details on the Online Referral form. One of our team will then call you back.
